Dear Parents/Guardians,

I hope this email finds you well and you are enjoying summer break! 

This is a reminder that in order for your child/children to be eligible to participate in athletics, there are requirements that need to be met first, one of which is listed below.

  • Complete annual preparticipation physical exam (see linked form).  You can submit the form on the Rank One website or scan and email a PDF directly to me.  Either are fine.  All 4 pages should be completed and sent in.
  • This physical exam is valid for 365 days starting from the day it was completed.

If you have already completed this, then you’re all done with this step of the eligibility process!

On  July 14th from 2:00pm - 4:00pm, we will be holding on campus preparticipation physical exams in the athletic training room.  If you were unable to secure an appointment before sports starting, you are welcome to come to the on campus event.  As a note, this is not a requirement to have the pre-participation physical exam completed at W+H. This is simply an additional resource for those that may be struggling to secure an appointment in a timely manner with their own doctor. The pre-participation physical exam can be completed by your child’s pediatrician as long as they are current with their cardiac training.  

The cost for the on campus exam will be $50 and a check made payable to The Wardlaw Hartridge School will be collected on the day of the exam. There is no extra fee for an EKG if one is indicated.

Dr. Heydinger is board-certified in both family and sports medicine and will be performing the physical exam. A pre-participation physical differs from an annual well visit in a few ways. The history and the physical exam put extra emphasis on the cardiac, pulmonary, and musculoskeletal systems and how they should properly function in athletes. Any student-athlete that has an abnormal cardiac exam during their pre-participation physical will receive an EKG the same day.  At the completion of the pre-participation physical the physicians will offer sports specific advice and injury prevention strategies if indicated. A pre-participation physical should not take the place of your child’s annual well visit with their primary care physician.

Physicals will take place between 2pm – 4pm. We understand the date and time of the physicals may conflict with your schedule. Therefore, parents/guardians who are unable to attend their athlete’s exam may designate a representative of their choosing, who is over 18 years of age. If you do not have a representative available, you may authorize an W+H Athletic Trainer to accompany your student-athlete. Please see the linked On Campus PPE Consent Form to help guide you on how to properly fill out the authorization form. If you plan on being present for the exam there is no need for this form to be completed.
